GLHA Covid Report for Teams (Greater London Hockey Association)

In order to keep a track and ensure we have everything needed for a game during this covid season. We are asking coaches to report to us if there are multiple covid cases, so we can ensure games can be rescheduled in a timely fashion if at all possible.
